Heh, yeah, it took me a while to figure it out too. It stands for "one true pairing." Apparently some people use it seriously, to refer to a pairing that they believe is the only acceptable/believable/good pairing in a fandom, or at least the only acceptable pairing for a character (for instance, "Hawkeye/BJ OTP" would exclude any possibility of Hawkeye/Trapper or Hawkeye/anyone else). But most of the people I know use it as a joke, like when they use it to refer to a totally crack pairing, like "Frank/Flagg OTP!" or something, or just to emphasize how much they like a particular pairing, without really meaning that it's the only "true" pairing.
There's also OT3, which means "one true threesome." XD
